Withdrawal Forms

The process to withdraw a student can be completed by following these steps: 

  1. K-8 Student: Complete the K-8 Grade Student Withdrawal Form
    9-12 Student: Complete the 9-12 Grade Student Withdrawal Form
     
  2. Return the completed withdrawal form to your child's school. You may email the form to:

    MMS (grades 7-8)whited@masonohioschools.com

  3. MCS property, including Chromebooks and Chromebook accessories, should be returned at your child's school .
