- signed deed
- signed lease agreement (all pages)
- signed settlement statement must be uploaded for us to make the address change
How to log into Final Forms, initiate a change of address, and request transportation changes.
This year parents are able to update student information in Final Forms. It is important that parents/guardians update information using their existing account information and should not create a new account when a phone number, email address, emergency contact or address change is needed.
Logging in to Final Forms
Login to final forms: https://mason-oh.finalforms.com/parents/login
Forgot Password Link: https://mason-oh.finalforms.com/parents/reset_password
Note: If you have forgotten your email address or password, click the “forgot your email?” or “forgot your password?” to receive an immediate email with a link to reset.
My Students
Once logged in, you'll see the “My students” page. Find your student at the bottom of the screen and click “Update forms”.
ADDRESS CHANGES:
On the left hand side, click CONTACT INFORMATION. This is where you will update your student’s address. You must also upload your proof of address change i.e. settlement statement, deed, signed lease agreement (both parties), or Affidavit II. Allow three days for the new bus to come to your new address.
Scroll down to each Guardian and click “Address: Click here if same as Student Home address”.
Transportation Change Request:
Next you will need to complete a transportation change request form. Click the link: “fill out this form” to be taken to a Google form which will go directly to the transportation department.
