If you already completed your student’s Emergency Medical Form using the personalized link you received before school began:
Use this form to update your student’s address, contact information, custody information, or other family information.
→ I HAVE NOT Completed the Emergency Medical Form
If you have NOT yet completed your student’s Emergency Medical Form:
- A personalized link was sent to you from noreplyenrollment@powerschool.com in July. You'll need to complete a form for each student because medical, emergency, & contact information may differ per student.
- Once you click the link in your email, you will need to create an account if you haven't already & proceed with the prompts to complete your forms per student (no app download is required).
